- Reviewed in the United States on September 19, 2024A powerful book because it share stories from those with stressful “front-line” occupations: in first responders in civilian as well as armed forces services. Stories they often were not comfortable sharing in the workplace or related support services because of concerns that their honesty might affect their career path or performance evaluations at their workplace. Major General Martin’s story was particularly compelling: forced resignation related to his bipolar condition. And years of hypomanic behavior including during the two years of psychiatric care he received after retirement ………all of this time before he was started on lithium therapy. Lithium was not an established frontline therapeutic option maybe because of deaths associated with its use initially, the requirement for prevention of deaths by close monitoring of blood levels of lithium, and maybe because awareness of this non-patentable therapeutic option is not supported by advertising from the pharmaceutical industry. Compelling question: could Major General Martin’s career have continued with even more achievements had the lithium therapy been started earlier in his career? Effective therapy can be challenging in a high stress environment such as occurs during war. Major General Martin’s courageous story in this stellar book is an important contribution to this ongoing controversy. Jack L Clausen, M;D.
